On my first turn in this game (turn IDs 429-439), I captured two territories but did not receive a card. There's also no "ended turn" message in the log.

I had the same log happen in a recent game. I was the next player to go, and my 'place', etc. buttons were all greyed out, but I don't know if the previous player didn't get a card. It would be interesting to hear from the next player to go and see if his buttons are all grey or not.

It's hard to say - for speed reasons the database is not transactional so the most likely cause is the process which handled your game action died somewhere between the game table commit and the game log commit.
