Have you ever forgotten to turn in a card set on your turn, and then got eliminated before your next move? Have you ever taken a territory you didn't know was a capital until you were unceremoniously eliminated by the next player? In this game of Five with Hugh, I marched blithely to my quick demise, never lifting a finger to defend myself. I probably should have read the rules, which clearly state in plain view on the right side of the board, "Top row fortifies to bottom row," but you know what? I didn't.
Also in a game of Five, I thought I had white reasonably under control, until I found out that the map has unlimited fortifies... every game I had played up to that point I had assumed only 2 fortifies. I'm not sure anyone can beat that! >_
My favorite awful mistake is when I'm on a big-stack elimination run but my finger stutters on the keyboard and I hit '3' an extra time, resulting in moving 3 armies out of like 30 rather than all of them, and stopping me dead in my tracks.
I do this probably about once a week.
I don't do it quite that often, but yes I have done that before and it does suck.
How about when you are tooling through an elimination run and all of the sudden the stupid interface breaks and won't let you select your units. You click them 10 times before you realize that you've landlocked your stack, and the border you thought you saw exists only in your head.
I spaced out my 2nd attack in a spies game that was required to obtain production!! I'm good for a spacey move at least once a week.
I find it really frustrating at first when I do these things, but then I become really amused by it.
I just placed all my armies in a country bordering the one I meant to put them in. Of course I owned all of the around that one already. Nothing to do but fortify and wait for the next turn.
I decided to join a game that Hugh was in.
After fortifying, I tried to go back into attack mode but it wasn't allowed. ..ON MY OWN BOARD!

I've found the only mistake worse than that is joining a game Blackdog is in. =/
Just played two entire games on the Rent is Due board and realized on my last turn that there is no attack border between the kitchen and hallway....

On Civil War one time I was about to eliminate a player, he had 2 territories left with 1 army each, and 5 cards, cards were around 70 or 80. His two countries appeared to border one another, and I attacked and transferred all 50 of my units into the first, which turned out to be bordered only by my territories.
In Rockem Sockem my opponent had been attacking my head each turn but I had been steadily resupplying the head while gaining bonuses elsewhere. When I felt I had enough of a bonus advantage, I placed all my units for a strike against his head but forgot to fortify my head which had 1 unit on it. The other guy did his usual head strike which executed before mine and he won the game.
My biggest mistake was to join WarGear.
The one that drives me crazy is when I have finally occupied an entire continent on a map that has abandon turned on with revert to neutral. Then I screw up and do a move all and empty out one of the territories. I do that all of the time on Dungeon.
